City of Grande Prairie launching Social Prescribing Initiative

The City of Grande Prairie is partnering with the Grande Prairie Primary Care Network to introduce a new Social Prescribing Initiative.

The initiative aims to allow care providers to make a social prescription to a link worker, a non-clinical employee working for senior-serving organizations, to find recreational and social programs.

Grande Prairie Mayor Jackie Clayton says building an inclusive caring community means developing and pursuing opportunities that enhance the quality of for all ages from youth to seniors.

GP PCN Executive Director Michel Haener says that are excited to see how the partnership will improve the health outcomes for Grande Prairie’s 55+ population.

To get a social prescription, Grande Prairie residents can book a self-referral service for social work on the GP PCN website.
